Federal Drug Testing Custody and Control Form (CCF) is used to document every urine collection required by the DOT drug testing program.
DOT urine collections must be documented on Federal CCF. CCF is a chain of 5 copies where each copy belongs to a different party.
1️⃣Copy 1 - Test Facility Copy (Laboratory Copy)
2️⃣Copy 2 – Medical Review Officer Copy
3️⃣Copy 3 – Collector Copy
4️⃣Copy 4 – Employer Copy
5️⃣Copy 5 – Donor Copy (Employee copy)
The driver must undergo testing for both drug and alcohol.
The driver should be tested for drugs only.
Note that alcohol testing could not be performed because the allowable testing period has passed.
According to regulation 49 CFR 382.303, if an Alcohol test required by this section is not administered within two hours following the accident, the employer must prepare and maintain a record stating the reasons for the delay. If the test is not administered within eight hours, the employer must cease attempts to administer the test and document the reasons.
Please note that the allowable time period to conduct either test has passed.
According to regulations 49 CFR 382.303:
Alcohol Tests:
if an Alcohol test required by this section is not administered within two hours following the accident, the employer must prepare and maintain a record stating the reasons for the delay. If the test is not administered within eight hours, the employer must cease attempts to administer the test and document the reasons.
Controlled Substance(Drug) Tests:
If not administered within 32 hours, the reason for delay must be documented, and no further attempts should be made.
In these circumstances, testing is not required.
